What is Dental Treatment?

Dental treatment is a complex of procedures aimed at eliminating pathological processes in the oral cavity and restoring the anatomical form and aesthetics of the teeth.

The strategy, scope, and duration of therapy are determined individually, depending on the type of problem and the complexity of the specific clinical case. For example, treatment options include:

  • caries cleaning + composite filling,
  • endodontic treatment + filling or crown.

Important

The dentists at the KiaDent Clinic of Author's Prosthetics focus on preserving teeth. Removal of nerves or the teeth themselves is resorted to only in extreme cases. Even the best prosthesis or implant cannot fully restore the chewing and aesthetic functions of natural teeth.

Sometimes patients think that simply placing a filling on a damaged tooth is enough. However, it is crucial to identify the cause of the problem and prevent further issues.

If necessary, our doctors recommend that patients undergo professional hygiene, treat gum diseases, other teeth, and the oral mucosa. Comprehensive dental treatment eliminates all foci of inflammation, reducing the risk of future caries, pulpitis, and periodontitis.


Indications and Contraindications for Dental Treatment

Indications for dental treatment:

  • bleeding, itching gums;
  • dark spots, enamel chips;
  • bad breath;
  • increased sensitivity to cold, hot, sweet, sour;
  • pain of varying intensity.

However, sometimes treatment is postponed until other health issues are resolved.

Relative contraindications

  • pregnancy, breastfeeding;
  • allergy to anesthetics, medications, treatment materials — medications are selected that do not cause an acute reaction;
  • viral, infectious diseases;
  • cancer recurrence or exacerbation of autoimmune pathology;
  • acute respiratory diseases;
  • kidney, liver, heart failure;
  • decompensated endocrine diseases.

Dental Treatment Stages

Caries

Caries is the destruction of enamel and dentin due to the action of pathogenic microorganisms.

Caries treatment methods:

  • In the chalky spot stage. We treat caries without drilling using ICON technology and remineralization.
  • Medium caries. We drill out the affected tissues. Fill the cavity with light-cured composites.
  • Deep caries. We remove carious tissues. Check pulp vitality and remove it if necessary. Isolate the pulp chamber with a liner. Restore the tooth with an inlay or crown.

Pulpitis

Pulpitis is the inflammation of the vascular-nerve bundle (pulp). It provides blood supply and nourishment to the tooth.

Conservative pulpitis therapy:

  • We reduce inflammation. Remove damaged tissues. Apply a microdressing with an antiseptic, anti-inflammatory solution. Place a temporary filling.
  • Check pulp condition (during the second visit). If the inflammation in the vascular-nerve bundle has subsided, we place a permanent filling during the second visit.

Surgical pulpitis treatment:

  • Assess pulp condition. Check nerve vitality using electrodontometry, thermal tests, and X-ray diagnostics. Clean out carious tissues. Open the pulp chamber, and remove the vascular bundle. Disinfect the root canals and fill them with temporary materials. Place a cement filling.
  • Root canal filling (during the second visit). Measure the root canals with an apex locator. Fill them with gutta-percha cones.
  • Tooth restoration (during the third visit). Place a composite filling, veneer, or crown.

Periodontitis

Periodontitis is inflammation of the root apex and surrounding tissues.

Therapeutic periodontitis treatment:

  • Root canal treatment. Remove carious tissues or old restorations. Extract the pulp or open previously filled canals using an endodontic drill. Disinfect them with antiseptic solutions and apply antibacterial agents. Place a temporary filling.
  • Root canal filling (after 10–14 days). Remove the temporary filling. Re-disinfect the canals with antiseptics. Measure the length and fill them with gutta-percha.
  • Tooth restoration (after 1–2 weeks). Perform a control X-ray. If there is no inflammation, place a light-cured filling or crown.

Surgical periodontitis treatment:

  • First option. Perform root apex resection through a lateral gum incision. This method is suitable for incisors and canines.
  • Second option. Extract the tooth that cannot be treated.

Granuloma

A granuloma is a capsule up to 5 mm in size filled with pus. It forms at the root apex due to advanced pulpitis, periodontitis, or poor endodontic treatment.

Conservative granuloma treatment:

Similar to periodontitis therapy. We treat root canals mechanically and antiseptically, and fill them. Additionally, we prescribe nonsteroidal anti-inflammatory drugs and antibiotics.

Surgical methods for granuloma removal:

  • Tooth-preserving surgeries. We perform root apex resection, enucleation of the cyst, and removal of the affected root along with the upper (crown) part.
  • Extraction. Done when the tooth cannot be saved. The tooth is extracted along with the granuloma. The socket is thoroughly cleaned and disinfected.

Cyst

A cyst is a benign growth filled with pus, larger than 0.5 cm. It develops from a granuloma.

Conservative and surgical methods for cyst treatment are the same as for granuloma therapy:

  • If the cyst size does not exceed 8 mm, we open and disinfect the root canals under antibiotic coverage. If the inflammation subsides, we fill the canals and restore the crown.
  • If the cyst is 8–10 mm or larger, we perform one of the tooth-preserving surgeries. If this is not possible, we remove the tooth along with the cyst.

Tooth Shape Restoration

At the KiaDent Clinic of Author's Prosthetics, we choose the method of tooth restoration depending on the degree of damage:

  • If the tooth is destroyed by 10–20%, we place a light-cured filling. We use premium light-polymer composites: Estelite and Gradia (Japan).
  • If the tooth is destroyed by 30–40%, we fix a dental inlay. This is a microprosthesis made of ceramics, which is much stronger than a light-cured filling. Used to restore posterior teeth.
  • If the tooth is

    destroyed by up to 40%, we install a veneer. A microprosthesis made of highly aesthetic and durable ceramics, suitable for restoring teeth in the smile zone.

  • If the tooth is destroyed by 50–60%, we cover the front or molar tooth with a crown. The prosthesis is made to individual jaw impressions from ceramics or zirconium dioxide.

About large composite restorations

Composite materials are very strong. They can restore teeth destroyed by 50–60%. In such cases, the tooth is pre-strengthened with a metal or fiberglass post.

However, large composite restorations are not long-lasting. Unlike prostheses and microprostheses, they gradually shrink horizontally and vertically. This disrupts the bonding between the filling and the tooth, allowing bacteria to enter the gap, causing caries, and eventually leading to the filling falling out.


Recovery and Prevention After Dental Treatment

To extend the life of fillings, crowns, and minimize the risk of new dental diseases, we recommend:

  • Avoid pigmented foods and drinks for a while. It is better to avoid them for 2 weeks after placing fillings or crowns to prevent color change.
  • Handle your own teeth and restorations with care. Avoid biting hard foods like candy, seeds, or nuts, and refrain from opening packaging or bottles with your teeth.
  • Maintain good oral hygiene. Brush your teeth at least twice a day and clean between teeth with floss or an irrigator.
  • Adjust your daily diet. Increase your intake of fresh vegetables and fruits. Reduce the consumption of sweets and sugary drinks.
  • Follow preventive measures. Visit your dentist every six months for early diagnosis of oral diseases. Undergo professional cleaning every 6–12 months at a dental clinic.

Questions and Answers

  • Is it normal for a tooth to hurt after caries treatment?

    Yes, it is normal to experience some pain after caries treatment. If the pain is mild, it should subside within 7–10 days. However, if the pain persists for several days or becomes severe, you should contact your doctor immediately.

  • Can teeth be treated during pregnancy?

    It depends on the stage of pregnancy and the condition of the mother and baby. The second trimester is considered the most favorable time for dental treatment. During the first and third trimesters, dental interventions are generally not recommended. However, for acute pain or infection, treatment may be performed after consultation with an obstetrician-gynecologist. The dentist will choose minimally invasive methods and safe anesthetics and medications.

  • What anesthetics are used at the clinic for dental treatment?

    Our clinic network uses modern, highly effective local anesthesia drugs. Before treatment, we conduct a thorough patient interview and perform allergy tests if necessary. This helps us choose the safest anesthetic for the specific clinical case.

  • Can pulpitis be treated at home?

    No. Pulpitis can only be treated endodontically in a dental clinic. The cavity must be cleaned of carious tissues, the nerve removed, and the canals disinfected and filled.

    At home, you can only temporarily relieve pain with medications, antiseptic rinses, or home remedies. However, if the pulp inflammation is not treated in time, it will progress into a purulent process: periodontitis, periostitis, or abscess.
