Indications and Contraindications for Tooth Extraction
Before extracting a tooth, the dental surgeon will perform a visual examination of the oral cavity and take an X-ray to determine the precise position of the tooth and confirm the impossibility of tooth-preserving treatment.
Indications
- acute purulent process in the tissues of the tooth and periodontium;
- odontogenic osteomyelitis, perimaxillary abscess;
- presence of a growing root cyst larger than 7 mm, which cannot be treated;
- extra teeth that injure soft tissues or interfere with tooth prosthetics;
- severe decay due to caries;
- significant mobility in periodontitis (3-4 degree);
- root fracture due to mechanical trauma;
- unviable treatment of granulomas or cysts at root apexes, etc.
Contraindications
- taking anticoagulants (the medication must be stopped 3-7 days before the procedure);
- 1st and 3rd trimesters of pregnancy;
- acute respiratory and viral diseases;
- severe cardiovascular conditions in the acute stage, etc.
Stages of Tooth Extraction

Simple Extraction
The problematic tooth is easily grasped with forceps (S-shaped for upper molars and curved for the lower jaw), carefully loosened, and extracted without surgical intervention in soft and bone tissues.

Complex Extraction
Soft tissues are incised, a mucoperiosteal flap is lifted, and sometimes part of the jawbone is removed to access the tooth. Using special tools, the root system is cut or separated, and the tooth is extracted in parts from the bone without damaging surrounding tissues.

Main Operation Steps
1. Step. Preparatory
Examination is conducted to assess the crown's integrity. Mobility and inflammation presence are determined. The patient is informed about the process and possible sensations.
2. Step. Anesthesia
Simple extractions are performed under local anesthesia. An injection is made into the gum, ensuring the procedure is pain-free.
3. Step. Extraction
The operation field is cleaned of food residues and disinfected. The gum is separated with a spatula or elevator. Forceps are applied and fixed. The tooth is loosened or rotated to break the periodontal ligament. After complete detachment from ligaments, the tooth is extracted outward or, less frequently, inward. The wound edges are approximated to minimize gaping, and a tampon is applied.
4. Step. Postoperative
To avoid a common complication after tooth extraction — alveolitis — all doctor's recommendations must be followed.

How to Prepare for Tooth Extraction?
- Eat a meal several hours before your visit to the dental surgeon, as you won’t be able to eat for 2 hours after the tooth root extraction procedure.
- Avoid alcohol for 2-3 days before the operation.
- Refrain from heavy physical exertion on the day of the operation.
- If you have a strong fear of the operation, take a mild sedative 1-2 hours before surgery.
After Tooth Extraction, It's Important To:
- refrain from eating and drinking for 2 hours;
- avoid rinsing your mouth to preserve the blood clot;
- refrain from eating spicy, salty, hot, or cold food for 24 hours;
- avoid physical exertion;
- abstain from sauna or bath visits for several days after the operation;
- avoid consuming alcoholic beverages.
Are Complications Possible After Tooth Extraction?
The most common complications include:
Dry Socket
Occurs due to the absence of a blood clot in the socket because of frequent mouth rinsing, eating solid food, or other violations of oral care rules after extraction. It is accompanied by severe pain and inflammation. A dentist should be consulted.
Alveolitis
An inflammatory process in the tooth socket due to medical errors or non-compliance with post-extraction recommendations. It is accompanied by swelling and mucosal inflammation, fever, and swollen lymph nodes. Antibiotics are required.
Nerve Damage (Paresthesia)
Manifests as numbness in the tongue, lips, parts of the cheeks, and chin. Minor nerve damage restores sensitivity within weeks, while severe damage requires consultation with a maxillofacial surgeon.
These issues are associated with unprofessional actions of the doctor or non-compliance with patient recommendations.

- Does extracting a damaged tooth hurt?
No. One or two injections into the gum, and the operation is completely painless. If necessary, the injection site is anesthetized with lidocaine gel.
- How to restore a tooth immediately after extraction?
Prolonged absence of a tooth leads to bone tissue atrophy, so it should be restored as soon as possible.

- How quickly does healing occur after surgery?
After a simple extraction, healing occurs relatively quickly:
- A blood clot forms in the socket;
- On the third day, the socket is covered with granulation tissue;
- By the 7th-8th day, a new layer of epithelium forms;
- On the 14th-18th day, bone tissue begins to form;
- Within a month, bone tissue grows from the edges to the center of the socket;
- By the third month, the bone completely fills the socket.
However, after a complex tooth extraction, the healing process is slower. A new layer of epithelium forms only on the 20th-30th day. By the 4th-6th month, the socket fills with bone tissue.
- How to behave after tooth extraction?
- Refrain from eating for at least 2 hours;
- Avoid heavy physical exertion, smoking, alcohol consumption, or hot baths for 2-4 days;
- Do not touch the socket with your tongue;
- Avoid brushing your teeth on the first day after the operation;
- Do not rinse the extraction area (unless prescribed by your doctor);
- Chew on the healthy side for several days to avoid injuring the gum;
- Take prescribed painkillers and antibiotics on time;
- Apply cold compresses every 2 hours for 15-20 minutes during the first day;
- Visit the doctor for a postoperative check-up in 2-3 days.
