What is Sinus Lift?
A sinus lift or subantral augmentation is a surgical procedure that increases the vertical bone volume of the upper jaw by raising the floor of the maxillary sinus and then placing bone graft material underneath it.
The maxillary sinus is a cavity located in the body of the upper jaw.
This procedure is a type of osteoplasty, allowing for the placement of an implant of the required length in an area with sufficient bone tissue volume to ensure high primary stability.
It may be required as a preparatory step for single tooth implantation, full implantation, all on 4, or all on 6.
Indications
- Resorption of the alveolar ridge (less than 6 mm to the floor of the maxillary sinus).
- Complex tooth extraction with a high risk of bone volume loss.
- Large maxillary sinus cavity.
Types of Sinus Lift
Closed (lateral)
Indications
Performed when the bone thickness is more than 4 mm, i.e., when only 2-3 mm of bone augmentation is needed.
How it is done
It is often performed immediately after tooth extraction.
A socket for the implant is drilled. Through it, the floor of the maxillary sinus is gently raised with a special instrument, and bone graft material is placed. The bone is covered with a special membrane. The gum is sutured.
The floor of the maxillary sinus can be raised using the balloon technique. A catheter with an elastic balloon at the end is inserted into the prepared hole. When air is pumped into it, it gently adjusts the position of the sinus floor without the risk of damage.

Open (vertical)
Indications
Performed when bone thickness is up to 4 mm, and it is necessary to increase the lost bone volume by 5-6 mm.
How it is done
A small hole is made on the lateral surface of the upper jaw, through which the mucosa is gently separated and bone graft material is added.

Open or closed sinus lift — which is better?
The choice is made by the maxillofacial surgeon depending on the clinical situation.
If it is necessary to augment up to 3 mm of bone, it is better to perform a closed sinus lift. This method is more gentle and does not require a long rehabilitation period. However, a closed sinus lift should only be performed by an experienced specialist, as there is a risk of damaging the sinus mucosa when lifting the floor of the maxillary sinus.
If more than 3 mm of bone volume needs to be increased, it is recommended to choose the open procedure.
Materials for Sinus Lift
Bone graft material creates a framework for the growth of the patient’s own bone. The most popular are:
Allogeneic
This is donor material. Its properties include fast osteogenesis, osteoconductivity, and no immune rejection.
Modern technologies allow for the testing and treatment of the bone, eliminating the risk of infection or disease transmission.
Xenogeneic
Bio-Oss — bone from animals (cattle, horses, or pigs), from which all organic substances have been removed to make it safe for transplantation to the patient.
The porous structure of these hard tissues is similar to that of human bone. It provides an optimal environment for vascularization and new bone adhesion.
Autogenous
Has properties similar to allogeneic material. The main difference is that the bone for transplantation is taken from the patient.
Autogenous bone is often mixed with bioactive glass (calcium-substituted silicon oxide).
During implantation, a gel layer rich in silica forms on the surface of the bioglass, and above it, calcium and phosphorus. This is necessary for the adhesion of collagen fibers and the formation of skeletal connective tissues.
Are only hard tissues transplanted during the procedure?
No. The bone graft is covered with a patch to protect it. The most commonly used membrane is Bio-Gide, which is derived from pig tissues.
Contraindications
Relative
Conditions that can be corrected:
- Cardiovascular diseases;
- Anticoagulant use;
- Poor blood clotting;
- Thyroid diseases;
- Allergic rhinitis;
- Chronic sinusitis;
- Bruxism;
- Malocclusion;
- Dental diseases (periodontitis, caries);
- Smoking;
- Ongoing chemotherapy.
Absolute
When transplantation is completely impossible:
- Decompensated diabetes mellitus;
- Chronic diseases related to poor blood clotting;
- Mental disorders.
Is it possible to avoid a sinus lift?
Dental implants are possible when there is sufficient bone tissue volume. In case of a deficiency, doctors may opt for alternative treatments, such as placing non-standard-sized implants (thin or short). However, this method carries risks. Thin artificial roots are prone to fractures, while short ones may not withstand chewing loads and could be rejected.
Recovery after Sinus Lift
Post-operative care is very important. Pain, swelling, and complications can be minimized by following the doctor's instructions.
What to do:
- Take painkillers and antibiotics prescribed by your doctor, in the exact dosage.
- Rinse your mouth with chlorhexidine or saline solution (4-5 times a day) for a week.
What not to do:
- Eat very hot food/drinks for the first 24 hours.
- Vigorously rinse your mouth or touch the area of the wound immediately after surgery.
- Brush the 2 adjacent teeth on both sides of the transplant area for 5 days.
- Engage in sports or visit saunas/baths for 1-2 weeks.
- Fly on a plane for 2-4 weeks.
- Smoke for 1-2 months.
Sinus Lift Steps
1. Step. Examination
The implant surgeon visually assesses the condition of the oral cavity. The results of CBCT (cone-beam computed tomography) are analyzed: the spatial configuration of the maxillary sinus is determined, its mucosa is evaluated, and pathologies (e.g., cysts) are identified.
2. Step. Oral cavity sanitation
Diagnosed dental diseases, such as caries, are treated. A comprehensive cleaning of teeth from plaque and tartar is performed.
3. Step. Preparation for surgical access
In the open technique, the implant surgeon makes an incision in the mucosa on the outer side of the alveolar ridge above the gum and drills a hole.
In the closed technique, an implantation bed is drilled in the socket, serving as access to the floor of the maxillary sinus.
4. Step. Bone graft placement
The sinus floor is lifted, and bone graft material is added. In an open sinus lift, the incision site is sutured and treated with an antiseptic.
5. Step. Implant placement
In an open sinus lift, the dental implant is placed only after 4-6 months, and a permanent crown is loaded another 4-6 months later.
In the closed technique, if there are no contraindications, a dental implant can be placed immediately.
Questions and Answers
- What complications can arise after a sinus lift?
This is a well-studied, predictable, and reliable bone grafting method. Complications such as sinusitis, Schneiderian membrane perforation in the sinus area, or bone graft rejection can occur only due to surgeon error or if the patient violates post-operative care instructions.
- Does swelling occur after the surgery?
As with any surgical intervention, swelling persists for 7 days after a sinus lift, with the peak subsiding by day 3-4.
Swelling can be reduced by applying a cold compress and treating the wound with an antiseptic. Medications to reduce swelling should only be taken as recommended by the treating physician.
If the swelling does not subside after 7 days, body temperature rises to subfebrile levels, or pain is not relieved by analgesics, you should consult your doctor.
- What should you avoid after surgery?
For the first 2-3 weeks after a sinus lift, patients are advised to avoid:
Sneezing while pinching the nose or closing the mouth.
Blowing the nose.
Flying on a plane.
Visiting saunas/baths.
Engaging in sports.
Drinking alcohol and smoking.
Drinking through a straw.
Eating hard, spicy, or hot food.
Chewing on the operated side.
- Why is smoking prohibited?
It is recommended to refrain from smoking for 1-2 months before and after the sinus lift.
Hot smoke and nicotine slow down blood flow in the mucosa, causing burns to soft tissues, which hinders cell nourishment and regeneration. They also create a favorable environment for pathogenic bacteria. As a result, inflammation can develop at the surgery site.
- What are the types of non
-standard implants?
1. Ultra-short rods, 5-6 mm long, allow for placement in a small bone volume. However, they do not distribute chewing loads adequately and are sensitive to tissue quality (they cannot be placed in porous bone).
2. Zygomatic implants (Zygoma/Zygomatic). Titanium rods longer than 40 mm are placed at an angle into the zygomatic bone. This method is used for critical bone tissue atrophy and complete tooth loss in the upper jaw.
