What Is a Wisdom Tooth?

Wisdom teeth are the third molars, also known as "eights."

They typically erupt last, between the ages of 20 and 25. Sometimes they don’t erupt and are called impacted teeth. They may also grow incorrectly relative to the dental arch (malpositioned). Third molars serve no functional purpose, do not participate in chewing, and do not affect bite formation.

Should Wisdom Teeth Be Removed?

Dentists agree that if there are no direct indications for removal, wisdom teeth should be treated and preserved. Here are a few reasons why:

First, eights can be used by prosthodontists for bridge placement, for example, when the sixth or seventh tooth is missing. They can also provide more reliable support for a removable denture.

Second, if the wisdom tooth is extracted, its antagonist may over-erupt due to the absence of chewing pressure.

Third, in rare cases, a healthy and properly shaped wisdom tooth can be used for replantation—it can replace a previously removed tooth. However, this requires pulpectomy of the wisdom tooth.

Indications for Removal

It is better to remove a wisdom tooth if it:

Interferes with orthodontic treatment to correct bite alignment
Causes crowding due to a lack of space for eruption
Exerts pressure on the seventh tooth, leading to its destruction
Has caused an inflamed gum flap (pericoronitis)
Is extensively damaged by caries or has complications such as pulpitis or periodontitis
Has an abscess at the root, risking jaw periostitis or osteomyelitis
Is abnormally positioned, injuring adjacent teeth or soft tissues
Has a (root cyst)
Is severely fractured due to trauma

Stages of Wisdom Tooth Extraction

The sequence of steps depends on the clinical situation.

Simple Extraction: Fully Erupted Wisdom Tooth with Properly Positioned Roots

1. Step
The dental surgeon visually examines the oral cavity and assesses the position of the wisdom tooth. A panoramic X-ray (orthopantomogram) may be taken if necessary.
2. Step
After administering a local anesthetic injection, the dentist uses forceps to grip the problematic third molar, gently loosens it, and removes it. The patient experiences no pain.
3. Step
The resulting socket is treated with an antiseptic. If needed, the socket is sutured. The procedure duration is up to 20 minutes.

Complex Extraction: Impacted or Malpositioned Wisdom Tooth

1. Step
Following a visual examination, the dental surgeon performs a panoramic X-ray (and targeted X-ray imaging if necessary). The surgeon evaluates the condition and position of the wisdom tooth roots.
2. Step
After treating the oral cavity with an antiseptic, the dentist makes a small incision in the gum under local anesthesia to access the problematic tooth. The wisdom tooth is carefully sectioned and removed in parts without causing pain.
3. Step
The socket is thoroughly irrigated, and sutures are placed.

Differences Between Upper and Lower Wisdom Tooth Extractions

The upper jaw is softer and more porous, making upper wisdom tooth extractions faster and simpler. However, the dentist must ensure that the maxillary sinuses are not damaged, as wisdom tooth roots may extend into them.

The lower jaw is denser, and wisdom teeth are more firmly anchored. Lower wisdom tooth roots are often irregularly shaped and may wrap around the mandibular nerve. Before extraction, a panoramic X-ray or even a CT scan is required to plan the operation and avoid complications.

Post-Extraction Care and Recovery

Apply cold compresses for the first few hours to reduce swelling. Do not use hot compresses, as they can worsen swelling.
Avoid rinsing your mouth for the first few days; instead, hold an antiseptic solution in your mouth for 20-30 seconds and gently spit it out.
Begin brushing your teeth the day after surgery, but use a soft brush and clean the teeth near the extraction site very gently to avoid irritation.
Chew food on the side opposite the extraction site.
Drink and eat only room-temperature food and beverages. Avoid hard and crunchy foods for the first 3-5 days.
Refrain from smoking for at least a week.
Limit physical activity for 5-7 days after the extraction.

Potential Complications After Extraction

Cheek Swelling

Swelling of the gum and cheek is a normal reaction following the extraction of an impacted wisdom tooth. Swollen lymph nodes and discomfort when swallowing may also occur. Typically, swelling subsides by the fourth day. If it persists, consult your dentist.

Alveolitis

Failure to follow post-operative instructions may lead to gum inflammation, with symptoms including:

Gum swelling and redness
Headache
Fever
Inflamed submandibular lymph nodes

Abscess

An infection may spread to the periosteum, causing inflammation. Symptoms include:

Gum swelling and pus formation
Sharp, shooting pain
High fever and body aches

If you notice signs of an abscess, consult your dentist immediately.

Damage to the Mandibular Nerve

This may result in paresthesia, causing impaired sensation, taste, or pain perception. In some cases, it may affect vision or hearing. Most often, it resolves within a few days but may require treatment in some cases.

Perforation of the Maxillary Sinus Floor

Improper planning during upper wisdom tooth extraction can damage the maxillary sinus. Symptoms include:

Whistling or gurgling sounds in the extraction socket
Bloody or yellowish nasal discharge
Nasal speech

Questions and Answers

When can I eat?
Do not eat or drink for the first 2-3 hours after surgery. Avoid hot, fibrous, or grainy foods for the first 2-3 days.
When can I drink alcohol?
Alcohol can be consumed only after 3-4 days following simple extraction or 7-8 days after complex extraction. Alcohol may cause bleeding from the extraction socket and interfere with prescribed antibiotics or anesthetics.
How long will the gum hurt after extraction?
Pain, difficulty swallowing, and swelling of the soft tissues are normal reactions to surgery. Pain should subside by day 7. If it persists beyond a week, contact your dentist.
How can I avoid complications after complex extraction?
In addition to standard recommendations, take the following medications as prescribed by your dentist:
Antihistamine: Reduces swelling of the soft tissues. It’s best taken before bedtime due to potential drowsiness.
Antibiotic: Prevents infection and helps manage pain.
Analgesic: Provides pain relief and has anti-inflammatory properties.
Does wisdom tooth extraction cause gaps or spacing?
No, this is a misconception. Two large molars are situated in front of the wisdom tooth, and their displacement is highly unlikely. Wisdom tooth removal does not lead to aesthetic issues.
