Features of Self-Ligating Braces
Self-ligating brace systems consist of:
- Memory-shaped archwire — a wire designed to exert tension to move teeth in the desired direction.
- Brackets — orthodontic attachments to secure the archwire to the dental arch.
These systems use advanced locks with flip-top caps (clips), which eliminate the need for traditional ligature connections. The doctor saves time by not needing to place a separate ligature on each bracket; the wire is simply inserted into the slots and snapped in place.
In self-ligating systems, two types of ligation (or securing the archwire in the bracket slots) are used:
- Passive: The bracket’s cap does not press on the archwire, providing gentler pressure on the dental arch, reducing the need for tooth extractions, and minimizing side effects.
- Active: The bracket’s cap presses the archwire into the bracket slot, increasing pressure on the teeth. Active ligation is necessary for severe bite defects or in the final stages of treatment to secure results.
Advantages and Disadvantages of Self-Ligating Systems Compared to Ligature Systems
Advantages
- Easier installation (patients spend less time in the clinic).
- 40% fewer follow-up visits (from 12–13 to 8–9).
- Brackets are less prone to staining, breakage, and easier to clean.
- More aesthetic and compact in the mouth.
- Available in a wide range of models (compact, aesthetic, lingual).
Disadvantages
- Higher cost.
Types of Self-Ligating Braces
Metal Braces
Made from stainless steel or, in rare cases, titanium or precious metals (gold, silver, platinum). Steel systems are unrivaled in terms of durability and clinical effectiveness.
Pros
- Correct any type of bite pathology.
- Rarely break.
- Affordable.
Cons
- Less aesthetic.
- Occasionally cause allergic reactions.
Aesthetic Ceramic and Sapphire Braces
Made from polycrystalline ceramics, resembling the natural color of tooth enamel. Sapphire systems are manufactured from monocrystalline aluminum oxide (synthetic sapphires).
Manufacturers offer various designs of aesthetic self-ligating braces. These systems can be entirely ceramic or hybrid (ceramic brackets with metal slots and caps coated for aesthetics).
Pros
- Provide enhanced aesthetics.
- Hypoallergenic.
Cons
- More expensive than metal systems.
- More fragile, prone to breakage.
- Not suitable for complex bite anomalies.
Are there lingual braces as well?
Aesthetic self-ligating braces also include lingual braces.
Unlike vestibular devices, these are fixed on the inner, non-visible side of the dental arch. Despite their clinical effectiveness, these systems are rarely used due to their high cost.
Steps of Installation
1. Step. Diagnosis and Oral Preparation
Examination by an orthodontist and radiological diagnostics. Treatment of caries, removal of damaged teeth, and professional oral hygiene. Taking impressions to create the orthodontic system.
2. Step. Installation of the System
Drying the tooth surfaces and isolating the gum tissue. Attaching brackets with dental photo-polymer adhesive. Fixing springs, hooks, and rings on molars. Installing and activating the archwires.
3. Step. Monitoring and Securing Results
Follow-up visits every 2–3 months for archwire replacements. Periodic evaluations every 4–6 months.
4. Step. Completion of Treatment
Removal of the brace system, professional oral cleaning, and installation of a retainer for 1–2 years or a retentive aligner.
How long does treatment with self-ligating systems take?
Treatment duration ranges from 1 to 3 years, depending on the clinical case. The minimum installation period is 12 months. The exact course duration is determined by the doctor after the examination and diagnostics.
Questions and Answers
- What should I choose: metal or ceramic?
If aesthetics are more important to the patient, we recommend ceramic or sapphire braces. Unfortunately, ceramic aesthetic systems are not recommended for correcting severe bite defects. In such cases, a lingual brace system is suitable.
- How should I care for self-ligating braces?
Proper care includes rinsing the mouth after meals, regular brushing with medium-hard toothbrushes and toothpaste, and cleaning hard-to-reach areas with interdental brushes or an irrigator.
During the active phase of treatment, avoid biting hard fruits and vegetables with front teeth. Cut hard foods into small pieces before chewing. Sticky foods, chewing gum, and extreme temperatures should also be avoided.
