Molar teeth include premolars (4th and 5th from the center) and molars (6th, 7th, and 8th).

They bear the brunt of the chewing load when eating. Their absence can lead to gastrointestinal diseases, sunken cheeks, and the appearance of wrinkles. Therefore, dentists recommend restoring them without delay.

High load on molar teeth

When restoring molars, higher requirements are placed on the primary stability of the implants because they must withstand very high loads. For this reason, thin and short implant models are not used.


What crowns are best for molar teeth?

When prosthetics are performed in the molar area, aesthetics are not as important (since these teeth are not visible when speaking or smiling), but strength is crucial.

The best option is a crown or bridge made of zirconium dioxide. A budget-friendly option is metal-ceramics. For anterior teeth, E-max all-ceramic crowns are ideal as they are nearly indistinguishable from natural teeth!

Implantation of lower molar teeth

Dense bone. The lower jaw has dense bone with a thick cortical layer. Even in the absence of a lower premolar or molar for several months, the bone rarely atrophies. Therefore, larger titanium posts can be placed without bone grafting.

Short osseointegration period. Implants in the lower jaw integrate faster, within 3 months. The better quality of the bone provides higher primary stability compared to the upper jaw.

Mandibular nerve. In the lower jaw area, a branch of the trigeminal nerve runs close by. It's essential to plan the operation carefully to avoid damaging the nerve during implant placement.


Implantation of upper molar teeth

Loose bone. The upper jaw bears less chewing load, making it less dense compared to the lower jaw, with most of it being spongy bone.

Long osseointegration period. Osseointegration of an implant in the upper jaw takes up to 6 months—twice as long as in the lower jaw. If a quicker restoration of an upper molar is required, implant systems with enhanced surface coatings (e.g., Astra Tech implants) can be used to accelerate the osseointegration process.

Maxillary sinuses. When implanting upper premolars and molars, it's important to consider the proximity of the sinus floor to the jawbone. After tooth loss, the bone quickly atrophies. To avoid damaging the sinus during implant placement, a sinus lift procedure may be needed to elevate the sinus floor and fill the resulting space with bone graft material.

Implantation of a single molar tooth

You can restore a missing premolar or molar by placing an implant and crown.

Implantation of several molars

Two options are available:

  1. Placing an implant in each missing tooth site, followed by crowns.
  2. Placing two implants and attaching a bridge to them.

Implant placement methods in the molar segment

Immediate

Indications:

  • a decayed or severely damaged tooth that cannot be treated;
  • mechanical trauma to the tooth, scheduled for extraction.

Involves implant placement immediately after the extraction of the problematic tooth. In the molar region, avoiding load on the implant is challenging, even if a temporary crown is excluded from the bite. Therefore, a "butterfly" prosthesis is used during the osseointegration period. Once the implant has fully integrated with the bone, it is loaded with a permanent crown.

Single-stage

Indications:

  • urgent or complex extraction of a premolar/molar (immediate implantation is contraindicated);
  • the absence of one or more molars;
  • refusal of bone grafting;
  • the need for rapid restoration of the dental arch.

An implant is screwed into the bone where the missing tooth was, and an immediate prosthesis is installed. After 3-6 months, once the artificial root has fused with the bone, it is loaded with a permanent crown.

Classic

Indications:

  • the absence of one or more teeth;
  • sufficient bone volume.

Classic two-stage implantation follows the protocol of delayed loading. First, the artificial root is placed, and then after 3-6 months of osseointegration, a permanent prosthesis is installed. During the healing period, an immediate "butterfly" prosthesis is used to eliminate aesthetic defects.

When is a sinus lift needed?

Indications include:

  • loss of bone volume after the extraction of an upper molar;
  • the maxillary sinus is anatomically close to the oral cavity;
  • deep alveolar process recess;
  • prolonged absence of a tooth in the posterior segment, leading to a lowering of the sinus floor;
  • enlargement of the sinus cavities with age.

An implantologist determines the need for a sinus lift during the patient’s diagnostic examination. A CT scan shows the exact height of the bone in the premolar/molar region and assesses its density.


Postoperative care after molar restoration

Following these rules during the rehabilitation period will accelerate recovery and help avoid complications:

  • Avoid disturbing the implant site with your tongue.
  • Avoid the gym, sauna, and bathhouses for 7-14 days after implantation.
  • For the first 4 days, use oral baths by holding antiseptic solutions in your mouth and gently spitting them out.
  • On day 5, start brushing your teeth twice a day with a soft-bristled toothbrush.
  • An irrigator can only be used 14-21 days after surgery.
  • Avoid electric toothbrushes until the implant is fully integrated (vibrations could loosen it and interfere with osseointegration).
  • For the first month, consume only soft, minced foods (avoid spicy, acidic, and hard foods).
  • Refrain from smoking for at least 2 weeks (tobacco smoke impairs tissue healing in the oral cavity and may cause implant rejection).
  • After a sinus lift, sneeze, cough, and blow your nose gently.
  • Take all medications prescribed by your doctor, including antibiotics.
